In rare cases when we use it to friends who meets after long time this sentence refers to "What work you are doing?" or "In what profession you are?'
En madtha iddhiya?? Is the singular form which is commonly used among the friends, where En madtha iddhira? is used to more than two people or Elder/Respectable persons.
Other persons answers this by saying what they are presently doing.
